MRCPCH FOP vs TAS — What Each Paper Tests
Your MRCPCH FOP-TAS Preparation starts here!
Exam Format
FOP (Foundation of Practice) is rooted in the basic and biomedical sciences that underpin paediatric medicine. It tests your understanding of:
- Anatomy & physiology — how the developing child's body is structured and functions
- Biochemistry & genetics — metabolic pathways, inheritance patterns, chromosomal disorders
- Pharmacology — drug mechanisms, dosing principles, side effects
- Pathology & microbiology — disease processes, organisms, immune responses
- Statistics & epidemiology — study design, data interpretation, screening concepts
Think of FOP as testing why things happen at a scientific level.
TAS (Theory and Science) bridges basic science with real clinical paediatric practice. It tests your ability to apply knowledge to:
- Clinical paediatric conditions — recognition, diagnosis, and management of common and serious childhood illnesses
- Neonatology — newborn care, prematurity, congenital conditions
- Child development & behaviour — developmental milestones, neurodevelopmental disorders
- Safeguarding & ethics — child protection principles and medico-legal awareness
- Interpretation of investigations — blood results, imaging, ECGs in a paediatric context
Think of TAS as testing what you do when a sick child is in front of you.
MRCPCH FOP (Applied Knowledge in Practice) — What It Tests
AKP is the most clinically advanced of the three written papers. It assumes you have already mastered the science (FOP) and clinical theory (TAS), and now tests whether you can function as a competent, decision-making paediatrician in real-world scenarios.
- Clinical decision-making — choosing the most appropriate next step in investigation or management under realistic time and resource constraints
- Prescribing & therapeutics — safe drug selection, dosing, recognising adverse effects and interactions in paediatric patients
- Emergency & acute paediatrics — prioritising and managing critically unwell children, including resuscitation scenarios
- Multisystem & complex cases — integrating findings across multiple specialties to reach a diagnosis or management plan
- Communication & professionalism — scenarios involving breaking bad news, consent, and working within a multidisciplinary team
- Governance, safety & ethics — clinical audit, error recognition, child protection, and ethical dilemmas in practice
- Evidence-based medicine — applying current guidelines (NICE, BNF for Children, RCPCH) to clinical decisions
| FOP | TAS | AKP | |
|---|---|---|---|
| Focus | Basic science | Clinical knowledge | Clinical judgement |
| Style | Recall-based | Knowledge application | Decision-making in context |

FAQS
MRCPCH Exam — Frequently Asked Questions
Find answers to the most common questions.
The MRCPCH (Membership of the Royal College of Paediatrics and Child Health) is a postgraduate medical qualification for doctors specialising in paediatrics. It is primarily aimed at paediatric trainees in the UK and international medical graduates seeking to demonstrate clinical competence in child health.
The MRCPCH consists of three written theory exams — Foundation of Practice (FOP), Theory and Science (TAS), and Applied Knowledge in Practice (AKP) — followed by the MRCPCH Clinical exam. All written parts must be passed before sitting the clinical.
FOP tests the basic sciences underpinning paediatric practice, including anatomy, physiology, pharmacology, and pathology, while TAS focuses on applying that knowledge to clinical paediatric scenarios. Both use a single-best-answer (SBA) format but differ in their depth and clinical orientation.
Each written paper (FOP and TAS) contains 70 SBA questions to be answered in two and a half hours. The AKP paper contains 100 questions across a longer sitting to reflect its broader applied clinical scope.
No, there is no negative marking in any of the MRCPCH written papers, so candidates are encouraged to attempt every question. Your score is based purely on the number of correct answers you select.
The pass mark is set using the Angoff method by a panel of examiners and is not fixed — it varies from diet to diet depending on question difficulty. This means the pass mark is adjusted to ensure fairness across different exam sittings.
The RCPCH allows a maximum of six attempts for each of the written components (FOP, TAS, and AKP). Candidates who exhaust all attempts without passing must apply for special consideration to continue.
The MRCPCH Clinical is an OSCE-style (Objective Structured Clinical Examination) assessment that evaluates a candidate's practical skills, communication, clinical reasoning, and interaction with real or simulated paediatric patients. It is considered the most challenging part of the qualification and requires thorough hands-on preparation.
Yes, the MRCPCH is open to international medical graduates worldwide, provided they hold a recognised primary medical degree and meet the RCPCH's eligibility criteria. Many IMGs from Pakistan, Egypt, Nigeria, and other countries pursue this qualification to advance their paediatric careers globally.
The timeline varies depending on individual preparation and attempt history, but most candidates complete all written papers and the clinical exam within two to four years. Structured study using high-yield resources, question banks, and clinical exposure significantly improves the chances of passing within a shorter timeframe.

I recently subscribed to the FOP TAS Course by PrepXperts Academy for my exam prep, and honestly, was totally worth it! The lectures are super easy to follow and make even tricky topics simple to understand. I only needed about an hour of self-prep besides the online sessions, which saved me a lot of time. Highly recommended for anyone preparing for FOP TAS - no need to waste money on complicated question banks when these lectures cover everything so clearly.
