It comprises two exams,

MRCPCH AKP (Applied Knowledge in Practice) — What It Tests
- Clinical decision-making — choosing the most appropriate next step in investigation or management under realistic time and resource constraints
- Prescribing & therapeutics — safe drug selection, dosing, recognising adverse effects and interactions in paediatric patients

The MRCPCH (Membership of the Royal College of Paediatrics and Child Health) is a postgraduate medical qualification for doctors specialising in paediatrics. It is primarily aimed at paediatric trainees in the UK and international medical graduates seeking to demonstrate clinical competence in child health.
The MRCPCH consists of three written theory exams — Foundation of Practice (FOP), Theory and Science (TAS), and Applied Knowledge in Practice (AKP) — followed by the MRCPCH Clinical exam. All written parts must be passed before sitting the clinical.
FOP tests the basic sciences underpinning paediatric practice, including anatomy, physiology, pharmacology, and pathology, while TAS focuses on applying that knowledge to clinical paediatric scenarios. Both use a single-best-answer (SBA) format but differ in their depth and clinical orientation.
Each written paper (FOP and TAS) contains 70 SBA questions to be answered in two and a half hours. The AKP paper contains 100 questions across a longer sitting to reflect its broader applied clinical scope.
No, there is no negative marking in any of the MRCPCH written papers, so candidates are encouraged to attempt every question. Your score is based purely on the number of correct answers you select.
The pass mark is set using the Angoff method by a panel of examiners and is not fixed — it varies from diet to diet depending on question difficulty. This means the pass mark is adjusted to ensure fairness across different exam sittings.
The RCPCH allows a maximum of six attempts for each of the written components (FOP, TAS, and AKP). Candidates who exhaust all attempts without passing must apply for special consideration to continue.
The MRCPCH Clinical is an OSCE-style (Objective Structured Clinical Examination) assessment that evaluates a candidate's practical skills, communication, clinical reasoning, and interaction with real or simulated paediatric patients. It is considered the most challenging part of the qualification and requires thorough hands-on preparation.
Yes, the MRCPCH is open to international medical graduates worldwide, provided they hold a recognised primary medical degree and meet the RCPCH's eligibility criteria. Many IMGs from Pakistan, Egypt, Nigeria, and other countries pursue this qualification to advance their paediatric careers globally.
The timeline varies depending on individual preparation and attempt history, but most candidates complete all written papers and the clinical exam within two to four years. The exam contained almost 80-85% exact statements that we have discussed in class. Having the concepts to attempt twisted statements made rest of the exam quite easy.
I gave exam in a chaotic state, while Shifting from one city to the other, i.e from karachi to Islamabad, along with preparation of FCPS 2 exam, it was not easy to focus and concentrate amid all these happenings, but Alhamdulillah and Solely by the grace of Allah, and with help of Dr Fatima, I was able to go through it somehow.
