The MRCEM (Membership of the Royal College of Emergency Medicine) is a postgraduate qualification for doctors pursuing a career in Emergency Medicine. It is awarded by the Royal College of Emergency Medicine (RCEM) in the UK.
The MRCEM consists of three parts: the Primary (a written SBA paper testing basic sciences), the Intermediate SBA (clinical knowledge), and the Final OSCE (a practical clinical skills assessment). All three must be passed to obtain the full membership.
Any registered medical practitioner with a primary medical qualification recognized by the RCEM is eligible to apply. There is no minimum period of clinical experience required specifically for the Primary, making it accessible early in training.
The MRCEM Primary consists of 60 Single Best Answer (SBA) questions to be completed in 90 minutes. The questions are based on basic medical sciences relevant to Emergency Medicine, including anatomy, physiology, and pharmacology.
The Intermediate SBA covers clinical Emergency Medicine topics such as acute presentations, trauma, paediatric emergencies, toxicology, and resuscitation. It is closely aligned with the RCEM curriculum and tests applied clinical knowledge rather than pure basic sciences.
The RCEM allows a maximum of six attempts for each part of the MRCEM examination. Candidates who exhaust all attempts without passing must apply to the RCEM for special consideration to sit again.
The pass mark is determined through a process called standard setting and varies slightly between sittings. It is not a fixed percentage, ensuring the mark reflects the difficulty of each particular paper.
The Final OSCE consists of a series of structured clinical stations, each lasting a set number of minutes, assessing skills such as history-taking, examination, procedural competency, and communication. It is designed to reflect real Emergency Department scenarios that a competent clinician would be expected to manage.
Once awarded, the MRCEM membership is a permanent qualification and does not expire. However, doctors practising in the UK are expected to maintain ongoing professional development through their respective colleges and the GMC revalidation process.
A structured study plan covering the RCEM curriculum, regular SBA practice questions, and focused revision of high-yield topics such as toxicology, trauma, and paediatric emergencies is strongly recommended. Joining a dedicated MRCEM preparation platform or study group can significantly improve both knowledge retention and exam confidence.
